If you would like to make either a conforming request, or a non-conforming request, of the architectural control committee, a subcommittee of your homeowner's association, please fill out the appropriate form below and bring it in, together with any pictures or drawings of your request, to the model center in your community.  If you are in a grand close-out community, and no model center is available, you may mail or drop-off your request to our corporate office at 828 E. Jefferson Boulevard, South Bend, Indiana 46617, or to any community model center at your convenience.
